Placing Orders

Contact customercare@bostonheart.eurofinsus.com if you are seeking to resupply kits, include your client ID in the email. The respective Area Sales Manager can also place orders for kits, marketing material, and other important items. 

Types of Kits

Patient Access Kit – Designed to ship directly to the patient, one set of tubes.

Small Kit – Designed to collect for two patients.

Large Kit – Designed to collect for six patients but can accommodate ten.

GI Balance Kit – Designed to analyze gut microbiome.

DBS Kit – Designed for Dried Blood Spot Collection.

Saliva Collection Kit – Used specifically for the Polygenic Risk Map.

Buccal Swab Kit – General genetic collection kit.

Note: Not all phlebotomists who work with Boston Heart carry kits on hand. Occasionally, the patient will need to bring a Patient Access Kit to the draw site. Ensure that it is stored properly before use.

Boston Heart has a nationwide network of phlebotomists and draw sites. To find the best solutions for any given practice, please email the respective Area Sales Manager for contacts.

 

Note: It is cost-effective to have a mobile phlebotomist come into the office when several patients are scheduled to get drawn.

Custom Phlebotomy Sheet

Unique to the Practice

Contact your Area Sales Rep to have a patient-facing flyer made for local phlebotomy options.

Provide the practice name and client ID within the email, and a Custom Phlebotomy Sheet will be created shortly.

Note: The flyer is designed to be emailed (or printed in-office) and shared with the patient for their convenience.
